PDF Form capture

Hello,
I've been using CMS for almost 2 years now with 10+ sites created so far.
What I would like to do is the following:
STEP 1:
Create a secure page for an end user to fill out a PDF forms, then hit on submit this form to the database and also to the printer (It is OK to hve two buttons, one to submit, one to print)
STEP 2:
Data captured in Step 1 would be used to generate additional forms, which I would pick and send to the printer.  Information captured in step one would help me automate form filling down the road as I would use info from that form to automatically fill and print multiple copies of different forms that contain data captured in step 1.

HINT:  For example banks use this kind of stuff when filling out loan paperwork.  They have the master form that contains name,SS#,address,phone #, employment, etc.  Then they choose what forms to send to the printer based on the nature of the loan.  In the end you get 3-4 different forms to sign.  Some of them contain all your info, some of them just your name and SS#, some of them just your name and address, etc.

Any ideas would be greatly appreciated.....
