Upgrading from 0.11.2 "Scarborough" to 0.12.1

Granted, the instructions for upgrading sounds simple, But...

when  I come to point 4. I am lost.

It says: "Navigate to /install/upgrade.php. That automatically makes any necessary changes in the database.

From where should I navigate? From CMS or from my hosting file manager? And then what? How do I 'Run' upgrade.php?
