FormBuilder - form does not work on one page

I have created a contact form and quote form on the following page

The forms are setup to email the results to me, but they do not work. 

If i place the form on another page they work but will not work using the contact us template page. 

I have also tried placing other forms from other pages on the contact us page and they do not work either - must be something with the page rather than the form.

can anyone shed some light on this please, bit stuck
