It seems that my issue is addressed and answered here. BUT, I don't understand the answer.
The answers states: Did you check "Ask in lost username?" on the properties you want included in the form? Edit your group in Frontend User Management, and make sure at least one property is checked as "Ask in lost username" I had the same problem until I found the solution here in the forums.
Where do you check "Ask in lost username"? Where do you find the properties? Sorry for not understanding. Thanks,

I got it: Apparently when you edit groups there is a check box that says" Ask in Lost Username" when you edit your group.
That box needs to be checked. For the box to be checked the field needs to be set as a required field.
