What most fail to realize is all pages are generated/made using the index.php in the root of the site, it has code to pull all content, style sheets, etc. etc. so if it looks like all your pages are infected then replacing the root index.php can go a long way in cleaning a site...
Note that both of them have index.php(1) which to me may mean there is more than one index.php in the root, always look thru the root and all folders comparing them to the unpacked cmsms folder on your computer to see if you have extra files not in the original package that you didn't put there, these may be the hackers files...
