installing form builder

i am new to cms made simple i need to use a form builder.

i have downloaded the form builder from
http://dev.cmsmadesimple.org/project/files/69

the FormBuilder-0.5.11.tar.gz file

can someone tell me how to install this form builder and implement this form into a page.

please advice.

Try installing via module manager and then read the module help section.
