Could not connect to database.

Hi I have a problem. First off:

I have installed cmsmadesimple-0.11.2.zip on my host in a sub folder (www/test/)
My host is running:
Operating system: Linux
Kernel version: 2.4.21-37.ELsmp
Machine Type: i686
Apache version: 1.3.34 (Unix)
PHP version: 4.4.1
MySQL version: 4.0.25-standard

I have successfully completed the first 3 steps of the Quick Install. When I get to the database portion I get this error message:

"Could not connect to database. Verify that username and password are correct, and that the user has access to the given database."

Ive checked and double checked the name of the dB file, Password, etc. I called the hosting company and double checked that all was correct. The host opened the port specified in the  Install program.

Im at my wits end. What am I doing wrong. Is there something I need to change because Im using a subfolder?

Would you care to enlighten us how you solved it? That way other people with the same problem may find the solution too, even if it was a embarrassing user error.
