Donation Form linked to Authorize.net

I inquired if anyone knew how to do this on the Product Support page, but I did not get any replies.  I am hoping just to subcontract the job out if anyone is willing to take it on.  My client is using CMSMS 1.4.1.

here is the request:

I am not sure where to ask this question - so hopefully Product Support is the right place to start.

I would like to make a simple donation form - one field will use a drop down box to select where the donation where go to, the amount field will have the donator put in the amount he would like to donate and the other radio/drop down field will select if this is a one-time or recurring donation.

Reading thru the forum, I found this CMSMS users site (http://forum.cmsmadesimple.org/index.php/topic,29603.0.html), which is an excellent example of what I am looking for (just go to the donate page).

Is there a module for doing this?

The form will be processed using Authorize.net.  The donator will have the option of either making a one time payment using Authorize.net's SIM method, or set up a recurring payment using Authorize.net's Automated Recurring Billing.  In either transmission, the payment is processed on Authorize.net's secured server.

I know this is a really old post, but did you ever get this solved? I'd also be willing to pay someone to take it on. I need a very similar form.
