feedback form error - please help

I've got the feedback form built so far so good. I add the Send Btn (email to set addresses) and get it to produce the template - which is perfect - but when testing it I only get the first 2 fields in the email. Then when I go back to edit the button, the template has indeed shrunk back to the two fields only.

Any help would be great as this site must be live in 12 hours as it is being advertised in a national paper. Help?

Have you upgraded to the latest version?

There was a bug in some previous versions where the template was limiting itself to 255 characters.

Even in that case, if you remove the template an email will be generated containing all fields/values, but it will not be formatted as nicely.
