NMS Option in Formbuilder?

NMS Option in Formbuilder?

This may have been asked before, but I can't seem to find it anywhere.

Is there a way to integrate the option of signing up for a NMS list on to a contact form made with Form Builder?

I just want to add a check box that says "Sign up for our E-Newsletter"
