Problem with Module Manager in 1.3.1

All my sites with 1.3.1 installed have a problem with the Module manager module, see attached image

Getting array before list of modules

Another issue is it possible to update from 1.3.1 to 1.4.1 cannot seem to find - diff files 1.31 - 1.4

Where can I download module manager 1.2.1, I cannot load modules using xml, (causes problems with the files on the server)
In the forge only 1.0 appears

Can some one direct me or create a link for me
