I am running version 1.4.1.
I have set the front end AND user preferences to french BUT the frontend stays in English...
What am I missing?

I'm running CMS Made Simple 1.4. I'm using PHP 4.4.1, and MySQL 5.1.6 This is all working with Apache 2.2.4 running on Linux with Mysql 5.0.27.

What links have you downloaded ?
May be you have the "light" (English only) core and miss the French language pack ?
From http://dev.cmsmadesimple.org/frs/?group_id=6 you should try cmsmadesimple-1.4.1-base.tar.gz with cmsmadesimple-1.4-langpack-fr_FR.tar.gz
