Form Builder - Problems with sending to named address (SOLVED)

I am trying to set up a form which when it is submitted will be emailed to my email address. There are two fields called "From name" for email: and "From address" for email: - can anyone tell me what I am supposed to put in these fields?

Second question: there is also a field called "Field Name:" which I have called 'Send' - does this field have to be entered onto the form?

I really need help with this - if I can't get the forms to work then it's Bye Bye CMS and back to a non-cmsms solution.

Here are the details of the the Send field that I have added to the form. Can anyone tell me what I need to do to get it to send to my email address? In particular the two 'From' fields that I have highlighted?

Field Name: Send

Field Type: *Email Results to set Address(es)

Field Validation: Automatic

Email Subject Line: (Smarty variables are valid in this field.)
