Help Please on Step one of install = Fails

Hello

Im having install problems.
I get here:
> http://www.communityoffaith.com/simple/ ... nstall.php
and its says "Thanks for installing CMS: CMS Made Simple" and "All of your tests show successful. It is time to setup your database."
I click the "Continue "button and get all the gunk below. Can someone help me please?
I set up a database called= cms
user: cms_user@localhost
pass= 'cms_pass

That is the strangest thing I've ever seen..

For some reason, PHP thinks that the include.php exists in a directory named: ../cmsmadesimple//home/comm1fai/public_html/simple

Is there any way that you could setup a phpinfo.php script?  (Basically, make a file called phpinfo.php somewhere and put in it, then open it in your browser)

I did put it in a DIR called simple, thats becasue I haver MAMBO in the root, but I dont like MAMBO. I want sumpin simple.
