I have noticed that CMSMS seems to be adding a dash in front of my page titles.

So far unable to get this to work without errors

I copied the menu template, made the changes, and then set the template to use the changed menu.

Good answers but I already uninstalled and reinstalled both FormBuilder and FormTools with no change.
Still the test mailer is successful but the forms submissions seem to go nowhere.
