Late reply.. sorry about that.
I ended up deleting the directory FormBuilder in /modules and the back end came back to normal. I haven't been able to install FormBuilder successfully though.
I might try updating CMSMS to the latest version (1.0.7 at the moment)

Hi all, I just installed the XML file version of Formbuilder into my CMS Made Simple. After it loaded, it went to this URL: /admin/listmodules.php This page was blank.
