UGLY FormBuilder!

My form is at

A few things I just don't understand:

1. How can I remove "Data" from the top of the form?
2. Is it possible to add a logo? I looked at the form template and it looks difficult?
3. WHERE is my form? I created it in formtools as a formbuilder form. It does not appear in the formbuilder module.
4. Is there a way to have this form look like the rest of the pages in the site, menus and all?

You don't make a form with 'formtools' but you make it with Formbuilder and then you'll see your form in FormBuilder.

Not how its working for me.

I add a form in FORM TOOLS and then click PUBLISH. That supposedly sends the form to Formbuilder as a php page.

However when I launch formbuilder they arent there. I find them in the Formbuilder/modules folder.

On editing the template I just cant find anything in the temlplate that causes "data" to appear at the top of the form.

I don't think you are talking about the "CMSMS compatible module named FormBuilder", but something completely different.
Unless you provide more data (like system info) for us to be able to know what are you talking about I'll move this to the Lounge board...
On the other hand if you ARE talking about FormBuilder 0.8.0 (made for CMSMS) it just won't accept nor recognize data from FORM TOOLS. Please read the module's help before posting. There is nothing there that suggests any kind of compatibility with FORM TOOLS.
