Formbuilder and uploads

I have a basic form with an upload field.
If everything works first time, it works perfectly.
If something the fails, the filename moves above the upload box, and looks like it is stored but in actual fact the file has not uploaded and then when the form works the file is still not uploaded.
I am guessing I am missing something in the form template but nothing stands out to me.
Any help gratefully received
