Looking for step by step question module?

I don't know if it exists but for a customer of mine I am designing and am planning to create the website with the help of CMSMS.

There's only one catch: my customer has asked me if I can make a questionaire in like this site: http://tinyurl.com/pp7ubc4

The website visitor needs to make a choice between a couple of subjects. Depending on the choice he makes, he'll be send to the next page and the next, etc. After making several choices he will be prompted with some input fields wich will sent the results of the answered questions to his e-mail address.

Does anybody know if such a module exists or how I can create this? I have years of experience in webdesign and html/css but php coding/development isn't my best field of expertise.

Haven't come across a module like that. Depending on how complex the flow diagram is I would consider just using Pages since each page appears to be linking to a number of others. Motto: Keep It Simple. :D

You will need FormBuilder for the final form.

Seperate pages would indeed be another option. Formbuilder is indeed the module I would use for submitting the personal info.

I ended up not have time to test it thoroughly, but was thinking about using this module for a similar project that didn't come through: http://dev.cmsmadesimple.org/projects/decisionmaker
I'm not even sure that it fits the bill, but if you try it and get it working, a feedback would be nice... :)

@Jo Morg: Thanks! At first sights this module looks very promising! I'll make sure I will give this a try and will keep you guys posted. BTW, do you know if the module is compatible with the most recent version of CMSMS?

@Rolf: Ofcourse, I live in Holland and one the main issues here (and ofcource in other countries) is the privacy statement of websites so no worries there.

@zaidcrowe: Do you have an example of this module somewhere? I would really like to see it in action.

I'm fairly sure that it was working with the CMSMS 1.11.x series. You will most probably need PHP 5.3+ to use it though as most (if not all) Calguy's modules require it.

I am not talking about privacy statements, disclaimers or so. I mean the way personal information is stored in your database! If your site gets hacked, all info is readable for the bad guys. You need to scramble the data before it is stored!

Ah oke, in that kind of way. There is no need for saving personal data in the DB because the questionaire serves as a tool for people to view wich situation fits their needs. After closing the questionaire all choices will be cleared.

If you are talking about the personal info, none of this information is stored. Every info given by the user is send to the webmaster, in this case my client.
