Parsing PHP in form

Hey guys, i have a crude landing page here : http://kensington-gardens.co.uk/index.php as you can see the PHP in the form isnt actually being parsed, rather displayed.

From the original index.php file (located here : http://kensington-gardens.co.uk/test/) you can see it works fine, any suggestions as to whats going on?

We need more info... but by default (and for security reasons) php parsing on the Smarty templates is turned off. The best way to use php on a template is by using UDT's.

You are aware that there is a very nice module to create forms?
