Problem about "Feedbackform" module

Hi,
I have tried to send mail using feedbackform module today, but I receive the email like this:
Form Name: Contact Form
Submission Date: Sun, 03 Sep 2006 11:20:49 -0600
Submission Host: www.casinobonuswatch.net
Submission Source: 61.230.4.73

Mail It!: $mail_it
Additional Comments, bug reports, etc.: $additional
I have tried 3 times, all the same. How do I solve this?
