Creating a libiability checkbox for FEU users

Is it possible to with cmsms (I have version 1.11.4), to create a liability checkbox for FEU's?

In my example I want the user to read the content on the page, check the box at the bottom of the page, before he or she has access to go further (i.e. the next page or a revealed global content block).

Has that been done already? I don't want to re-invent the wheel if this is a normal feature that has been implemented already.

Are you using SelfRegistration? I'm assuming that you can make fields mandatory? Can you make a custom field that's a checkbox that's mandatory?

With my CMS 1.11.4, system, I downloaded FrontEndUsers 1.21.11, and Self-Registration 1.7.8, but I can't figure out how to set my own custom form with self-registration (I am so use to using Form Builder).

For example, how do add a required field for phone number
Add a check-box to the form, and making it required to check it before the form is submitted?

Last,
is self-registration information actually stored anywhere in CMSMS so that admin users can see later? (i.e. each custom field value they filled out or checked?)
