Back-End distorted view

There's an attached file to this post, please kindly view it to let me know what to do. The back-end of a website I'm working on gave this distorted view after I tried to install FormBuilder. Waht should I do to restore it back to normal.

Most times that comes from installing something that doesn't work or is for a different version of CMSMS then what you have, please be a bit more descriptive in what you did to get to this point...

I suspect the max_upload_size setting is too low
