Problems logging on after upgrading to 1.9.4.1

Hi

I have several installations of CMSMS which are working fine on my host. I have upgraded one of them to 1.9.4.1 and this one (only) gives me a blank screen after logging on. (500 error).

...login.php is still showing in the URL when the error occurs, and if I change the URL to finish admin/, I can still get in.

I contacted my most who didn't want to investigate and blame my config.ini settings: so I've tried adjust output buffering, memory limit and upload size (as suggested by the wiki), but with no luck.

Anyone any idea if this is a permissions issue with 1.9.4.1, or how I need to adjust config.ini, or what else could be causing this??

My system info on the installation now reads:

CMS Version
1.9.4.1

then only the following have exclamation marks, all the others are ticks:

0

disable_functions in PHP (disable_functions):
passthru, proc_close, proc_get_status, proc_nice, proc_open, proc_terminate, shell_exec, system, apache_note, apache_setenv, closelog, debugger_off, debugger_on, define_syslog_variables, openlog, syslog, popen, pclose


Would be very grateful for any clues please - my users can't use this installation till I fix this

I have a more useful error message: has anyone had this?

Fatal error: Call to a member function Name() on a non-object in /usr/local/pem/vhosts/102179/webspace/httpdocs/HODCMSComm/lib/classes/class.usertagoperations.inc.php(158) : eval()'d code on line 9
