file manager in 1.9

Having random file manager issues in 1.9. When moving or deleting files, it (apparently) does the process, but then goes to a blank screen. In the address bar there is a module_error= at the end of the url, but no actual error.
Just recently after this error, it actually deleted the wrong files, including some system files. The site is now down with the message "Attempt to use ADODB from outside of CMS". I checked for missing files, and the only one I could see was version.php which I replaced from a different site. Didn't help.

replacing version.php from the svn fixed the issue I was having, but I can reproduce the file manager problems.
