Looking at the Uploads Module's help page, down at the bottom there is the folowing text:

(optional) fieldname="" - --Add Me - module:Uploads string:param_fieldname--
(optional) fieldval="" - --Add Me - module:Uploads string:param_fieldval--
(optional) fieldname2="" - --Add Me - module:Uploads string:param_fieldname--
(optional) fieldval2="" - --Add Me - module:Uploads string:param_fieldval--

Is this how the author wanted this to look? Is there something missing in the PHP code?

Those are missing lang. strings, please make a BR in forge...
