I tried searching it everywhere (google, documentation, forum), bit it seems there is no answer

The question is how to customize Forn template? Where I can find description of the smarty tags used in template?
Are there any extra templates to download?

Then: lots of examples in the forum...

What you mean "customize"? The way how look's your form is use css stylesheet. You can figure any element like textarea, input type [text, password], fieldset, label etc. Default css stylesheet you can find in your stylesheet, you can edit it and make any change to customizing your form.

Other way is customizing smarty, like wrote you earlier JanB
