Advanced form with back-end user interface.

Hello,
I need advanced form - it should be with Field relations. What i meen: When you click on one of the fields, the info of the second field to be related with the choosen option.... etc. for the rest of the fields. They are about 10 or close...number of fields.
I can give an example in more details if someone wants to start develop the module... I say module, cause it should be out of the other modules, and the back-end interface for entering info should been clear and simple. The back-end user\or moderator\ need to reedit the info in the fields and arrange them\the relations\ as it need.
Field1 \option1, option2\
Field2 \depends of the field1 options\
Field3 \depends of the field2 options\
.... etc.
The visitor of the web site will enter and choose the option \this is kind of registration form\ and when is done... all this will be send to email... like a Contacts form....\captha too\
