FormBuilder submit error (cmsms 1.6)

Since I upgraded to CMSMS 1.6 with FormBuilder 0.5.11 I'm getting this error when submitting the form (with or without any input).
Whats wrong?

I just downgraded to cmsms v.1.5.4 and the error still occurs??
Could it be a php update of mod_security update on the server site (hosting company) that they didn't inform me of?

I now updated to CMSMS 1.6.6. and the error still occurs.
What can I do?
