If you read my other thread, I asked if it was possible to create mysite.com/rby/index/ and mysite.com/gsc/index and have them function as separate pages. I was told this is not possible because the page alias is unique.

After looking at Drupal and Joomla, it struck me at how simple CMSMS is and how efficient it is in handling page queries so my ideas of using another CMS have been abandoned.

Which leaves me with the question: how difficult would it be to modify CMSMS so that separate page hierarchies can be used?

why do you need to have the /index/ on the end? if you are comparing it to standard html somedir/ is the same as somedir/index.html

In other words, please explain more  your need for this.
