Form Builder 0.5.11 BUG? NOT SENDING + XHTML and CSS Problem ...

Hello,
I have installed Form Builder 0.5.11 and now it fails to send messages and validate any page that is using "form", this is what I get back:
Sorry! This document can not be checked.
500 Internal Server Error
Before I was using Form Builder 0.5.5 and everything was just fine...
How to make new Form Builder to validate ...?
