problem in application

Hello,
I am using cmsmadesimple 1.5.2 .I have installed the necessary modules .
I am working on a site where i have my own index page where i want to add a login form
from existing modules of cmsmadesimple.
I am trying it from userfrontend ,templates .. and other relevant modules but in vain.
Pls, help me by providing step by step detail that i should do.
