Feedback form and news box(css) - Pays $30 USD

Hi,
Here's what I need done.
#1 - Need a form created that is similar to the one here:

The "form builder" module within CMS Made Simple should be used for this.
The form will be located on this page: (It also needs to match the design of the rest of the site).

#2 - Need a "block / news" added. Probably just some CSS koding. Example image below.

I will pay $30 USD(PayPal) for this. My budget is limited so I am afraid I can not pay more.
