Team sheet - any ideas

Has anybody tried developing an interactive "team" sheet.
I am putting together a website for my local swimming club. The coaches would like to be able to post a team sheet, that then requires the swimmers to respond indicating if they are available or not. I could just set up a few content blocks for them to over type but this wouldn't necessarily allow the swimmers to interact with the list. Ideally if they could check their names in the "front end" and then they get greyed out or highlighted in some way, as well as sending an email to the coach.
Is form builder the way to go? I was wondering if I could integrate this with the calendar/events module?
