index.php hacked

Does anybody know how to prevent the index.php file from getting hacked?
I have the CHMOD set to 644 and they were still able to replace everything in it that redirected to a website far far away.

This is the 3rd time this website has been hacked,

I have done the following and it still gets hacked into:

1st - removed the CMS version
2nd - upgraded to latest release
3rd - set CHMOD to 775 on modules and uploads
4th - set CHMOD to 444 on config.php
5th - set CHMOD to 755 on temp
6th - created .htaccess file in root directory and lib directory (following instructions from http://forum.cmsmadesimple.org/index.ph ... 0.new.html)

Any suggestions would be GREATLY appreciated!!!

Either someone has your password, or the server you're on has been compromised. You should contact your hosting company.
