CMS Made Simple Forums

I have installed all other modules with success but for some reason I can't get the banner module to install.  It says it installed but for some reason when I click on Manage Banners I see the three buttons Categories, Banners and Preferences but I only see a blank blue screen under them.  I notice at the bottom of the page I see a Javascript Error that says Error on Page.  It says:

Does anyone have any ideas why this module isn't working.  I checked the permissions on the module.
