CMSMS pages full of weird symbols

I've been working with CMSMS for half a year and it's my favorite cms to work with because it's simple, and gives me a sense of control. However, I'm having a problem that causes me to pull out my last hairs of my head. My site keeps giving - at random - pages that contain weird symbols; special characters that have been converted the wrong way. I don't know on how many places I've implemented the charset utf-8; on the templates, in the php.ini, in the .htaccess, in php files but it keeps giving me these pages at random. One moment they are okay, the other moment they are not. I've cleared browser's and cmsms's cache, changed browsers, looked in the databases and set them to UTF-8 Unicode. Nothing seems to work. Only the CMSMS pages are affected, so I assume it must be something there. It appears on regular cmsms content pages as well as the admin pages.
CMSMS version: 1.1.3.1
PHP version: 4.4.7
Mysql Version: 4.1.22
