Problem with Search <form ... /form> code generation and (possibly?) templates

Hi there
One of my pages is producing different code for the Search Form from all the others. The page also will not show its Page Block in the correct place. It uses the same template as all the other pages in its class that are working.
The search form code in View Page Source of the problem page is as follows:
For all the other pages, the variable names beginning with m5 (above) all begin with m4. (apologies it is not in code block mode but then the red doesn't show up)
Does anyone have any idea why there is a difference? It causes problems because I'm using the id value to set the search input box appearance. I can change the css code to include the m5searchinput id name, but it doesn't explain what is going on. It also doesn't explain why the Page Block for that page doesn't show up. Page Block location on my pages are controlled by the template -- if I call the pages's own Page Block from the page's content then it shows up there, so it does exist.
