[OK] I can't create a new fiel in form builder

Hello,
I would like created a form, I already used form builder on another site and another server, but this time ,there are nothing dislpayed when I click on “add new field” (I have the template cmsms, the title form builder with the link for help and that's all). I gave all the right, to the files and the user of database.
I forgot something?
precision: cmsms 1.0.8 “Kona”, form builder 0.2
