I'm afraid you will need the apache logs to solve this...
Maybe you can get some debug information by using the $smarty variable (http://www.smarty.net/docsv2/en/languag ... smarty.tpl) and then read several relevant value from php's $_SERVER (http://php.net/manual/en/reserved.variables.server.php)
e.g. $smarty.server.REQUEST_URI etc
