on the wysiwyg when editing a page, if i click on insert image button im getting a 404

The requested URL /modules/TinyMCE/tinymce/jscripts/tiny_mce/plugins/advimage/image.htm was not found on this server.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

I looked at the source files, and it isn't there.  I tried downloading it again, but image.htm is not present.
Couldn't find on the forums if this had been addressed, so apologies beforehand if it has.

this was originally on the newest release version0.10 stable. 

any help would be usefull,

What ever happened to this bug? Because I have installed Version 1.1 on our hosting web server and everything works great except when adding/editing image button on TinyMCE wysiwyg. When selected I get the same error as mentioned before except for the file image.htm does exist, even though is says it doesn't. Could it be folder permissions or what? Uploads permissions 777. Uploads/images 777. I checked to see if the advimage module was specified in functions.js file for TinyMCE and it is specified correctly. Thanks for your help in advance. Would appreciate any insight on this issue.

NOT FOUND!
The requested URL was not found on server.
I can guarantee that the file exists on server.

The 404 error when clicking the HTML code button.

Both our pop up windows and both are form TinyMCE. I have downloaded FCKeditorX and now I have deactivated TinyMCE and activated and selected in User Preferences FCKeditorX and all seems to work fine. What am I missing? Why would TinyMCE be the default if it doesn't work? How can we get TinyMCE 2.2 to work? Thanks in advanced, would appreciate any help.  :)
