More comments fields in gallery module

Hello, I need some help creating more comments fields in the gallery module. I've managed to get the comments to show on the image (got that tip from this forum), and i can't find a solution to this specific problem here. What I need is to have the title field, comment field and another comment field. Is it also possible to change the names? from title to Ref. for example and Comments to Dimensions:

CMS 1.10.3
latest gallery version
slimbox template for the gallery
