Back up DB and all folders/files, then if using FTP unpack 1.9.4.1 and upload it overwriting all, go to site.com/install/upgrade.php and follow the prompts you may need to change permissions on config.php to 666 so it can be written to...

Re: First Time upgrading.. from 1.8.2 to 1.9.4.1
Try duplicating your site in a side folder (copy the database as well). Make sure you change the paths and the database credentials in config.php.
Then replace the CMS files with the ones from 1.9.4.1, either full or diff (if available), and when installing, tell it to not overwrite the database/install default content/whatever. Once you're happy with the results, either upgrade the original install, or move the one you've already upgraded into the main folder, edit your config.php to reflect the new path, and you're done.
I never upgrade a live site.

@Wishbone
Going from 1.8.2 to 1.9* you can't use diff files...
On upgrade you don't run install but /install/upgrade.php...
