FormBuilder and PayPal

Hi all,

I am on CMSMS 1.6.5

I need to create a page that allows people to donate funds through PayPal. However, since this is for political contributions, we have to collect data from the person before they can contribute. Could anyone advise how I would create a form that does the following:

1. User clicks donate, opens a page for entering their information
2. Name, State of Residence, Name of their employer (we are thinking no address is needed since if donated through PayPal they get their receipt through PayPal)
3. Verbage with statements per election rules, with a checkbox for the donor to say they have read them
4.  Once the above information is entered, the person can now click PAY via Paypal
5. The information is saved to a location we can access (an Excel spreadsheet? I don't know)
6. Donor donates through paypal and returns to our site.
