All system info posted below.
I tested the mailer settings in CMSMailer module and I receive the test message. I also tested the Mail Settings in Setting - Global Settings and it too worked. Both are set to smtp.
I have created a generic form for testing, but when I submit the form, it does not submit.
I'm not sure what to do at this point.. please help!
Also, does Formbuilder use CMSMailer module or does it use the Mail Settings in Settings - Global Settings?

Check who the form is sending the results 'from'. If you're using a 'from' disposition, that could be part of it - your server may not allow mail to be sent from addresses outside of your domain.
Remove that field and make sure it's from a valid email address in your domain, ideally the one you're using in your smtp setup.
FB does use your CMS mail settings, but can override some of them, like the from address, depending upon your form.

The domain is cornerballoonshoppe.com and the from address is admin@cornerballoonshoppe.com.
I changed the from address to root@localhost.localdomain, which is the same as another domain I have on the same host and the test works, but the actual submission does not. Another thing I'm noticing is the submission template is not displaying.

I don't have direct access to the logs, but I called my host and they were watching the log files as we tested the form. Nothing is being written to the log files when submitting. Only when submitting a test from CMSMailer.
Note: Since our last correspondence I did upgrade to CMSMS 2.1.6. Here is the latest system info:

Probably need to change your error reporting level then, and have your host give you access to the logs (or find a host that will). What's the error that's going into the log when you use cmsmailer?

What exactly does this mean?I have created a generic form for testing, but when I submit the form, it does not submit.
When you click the submit button nothing happens? or does the page just reload to the same page/form but a POST actually takes place.
things to check include:
a: Ensure that the HTML validates.
b: Disable javascript and see if it works then (process of elimination).
c: Check for javascript errors (a script on the page may be broken OR may be interfering with the form).
d: Watch the network tab as you hit the submit button, ensure that a POST actually takes place.
e: Look at error logs (try debug mode).

I'm no longer using a generic form so you can ignore that comment.
a. page validates.
b. disbaled js and no change.
c. There is a javascript error on the console that says.
d. No post in Network tab when clicking submit.
e. tried debug mode, nothing out of the norm. Server logs aren't capturing error because it's not processing.
