error when using Formbuilder and CMSmailer

I am setting up a form using Form Builder, and I got an error when testing it. The first problem, I fixed... I was able to find an answer to here:

I changed the code on line 44 to match. and then tried to send another email using the form.

This time, I get another error:

Error: at line 127 in file ... /lib/smarty/sysplugins/smarty_internal_templatebase.php:

message: Unable to load template module_db_tpl 'FormBuilder;fb_2'

Any thoughts?
