Help with Module Writing Tutorial on CMSMS 2.2.3.1

Help with Module Writing Tutorial on CMSMS 2.2.3.1

I'm following the module writing guide. I've made sure the cases of the folder, Holidays.module and the class all match. However I can't see my module in the Extensions folder and if I try to look at Module Manager it doesn't display properly. If I remove the Holidays folder, Module Manager displays properly again.
What do I need to do in 2.2.3.1 so that the example module works?

The screen just shows the Notice para and the Use at Your Own Risk para in black with no menus, no sidebar etc., as in the attached screenshot.

check PHP error log, or check your code online, e.g.
