Simple voting counter to Company Directory

Hi,
Need simple like / voting counter to Company directory.
1) To detail template placed link (thumb image) and counter for voting results
2) When link is clicked, counter +1 to that company
3) Same user can not vote twice (cookies)
4) Voting results must be generated to summary template
5) Everything must be styleable with CSS
6) Option: Admin can see voters IP addresses (from database if easier)
