Database Error When Adding Banner When Hosted on Windows Topic is solved

I've gotten CMSMS up and running on Windows host, but I cannot seem to get the banner module working (I have it working on a Linux host).  I can successfully upload banners using the Uploads module and I can successfully create Banner categories.  When I attempt to add a banner, however, it fails with the simple error:

Error: Database Error!

Using phpMyAdmin, I have confirmed that the banner related tables are in the database.

A further test using phpMyAdmin indicates that I can manually insert the approprate data into the banners table and the module displays banners as should be expected.  This, of course, is not acceptable for the client.

Does anyone know what may be causing the issue and how I might fix it?
