 Post subject: Fickle Forms, Making radio buttons and checkboxes "sticky"

So, after a long absence, I'm back -- with more form stuff.  I have this rather elaborate form that has text fields, some required, some not, a radio button with three options and a whole bunch of checkboxes.  To see the form in action it's at: http://uniquelyyours.selfip.com/index.php?page=request-for-quote.  When a submitter leaves a required field empty the form displays error messages with the form so the fields can be filled.  All the text boxes display data already entered just as they should.  But the radio button field and none of the checkboxes retain their data. 

I have a feeling that once again, the solution is a simple one that I'm not seeing.  If someone could tell me what I'm leaving out, I'd be very grateful!  Here is the actual code:

I fail to see how this is in any way remotely related to cms made simple, OR to layout and design.
